I hereby release Rhythm Enterprises LV LLC, DBA Rhythm Nation Dance and Fitness Studio (hereinafter referred to as "the Studio"), its instructors, independent contractors, employees, volunteers, and all other associates from any liability for harm, theft, or injury that may be suffered by me, my child(ren), or any family member while traveling to, from, or during participation in any dance, fitness, or related activities at the Studio.
I acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising from active participation in dance and/or fitness classes, training, rehearsals, performances, or any other related activities. I understand that such activities carry inherent risks, including but not limited to slips, falls, muscle strains, cardiovascular risks, and other injuries, and I assume full responsibility for my participation or that of my child(ren).
I acknowledge the contagious nature of COVID-19 and other illnesses and voluntarily assume the risk that I and/or my child(ren) may be exposed to or infected while attending and participating in Studio activities. I understand that such exposure or infection may result in personal injury, illness, permanent disability, or death. I acknowledge that this risk may result from the actions, omissions, or negligence of myself and others, including but not limited to, employees, independent contractors, volunteers, and fellow participants. I hereby release the Studio, its instructors, independent contractors, employees, and all associates from any liability related to COVID-19 or other contagious diseases.
I understand that there are no refunds for registration fees, costumes, recital fees, classes, or fitness memberships. If a dance or fitness class is canceled due to weather, illness, or issues arising from COVID-19 or other emergencies, I acknowledge that no refunds will be issued for these missed classes or sessions.
I acknowledge and agree to abide by all Studio policies and procedures. I understand that this release form must be signed before attending any dance or fitness class.
I grant permission for the Studio to use photos and/or videos of myself or my child(ren) for advertising and promotional purposes, including but not limited to print materials, promotional videos, social media, and the Studio’s website.
Tuition & Fees
Tuition and/or membership fees are due by the 1st of each month. A $20 late fee will be added if payment is not received on time.
This late fee will increase by $5 each additional day until the balance is paid in full.
If a payment is 30 days overdue, a $50 fee will be applied to the account.
If a payment is more than 4 months overdue, the account will be referred to a collection agency.
A convenience fee will be applied to all credit and debit card transactions. This fee will apply each time a card is used.
